چکیده

Penetrating cardiac injuries are commonly instantaneously lethal. However, numerous delayed manifestations of cardiac injury, including traumatic ventricular septal defect, ventricular aneurysms, right pulmonary artery to left atrial fistula, valvular regurgitation, and aorto-right ventricular fistula, are all described in the literature. Here, we present a case of aorto-right ventricular fistula following a stab wound to the chest. Epidemiology, diagnosis, imaging, and management of penetrating cardiac injury and intracardiac fistulas are discussed. Correspondence to: James R. Yon, MD, 1900 W. Undetected Aorto-RV Fistula With Aortic Valve Injury and Delayed Cardiac Tamponade following a Chest Stab Wound: A Case Report

INTRODUCTION Although a few patients will survive after penetrating cardiac injuries, some of them may have unnoticeable intracardiac injuries. The combination of aorto-right ventricular fistula with aortic valve injury is rare. CASE PRESENTATION A 19 year-old man referred with an aorto-right ventricular fistula accompanied with aortic regurgitation and delayed tamponade following a stab in t...

Elective correction of intracardiac lesions resulting from penetrating wounds of the heart.

Controversy exists regarding the timing and technique of total correction of traumatic intracardiac lesions. Five patients with penetrating wounds of the heart received emergency treatment aimed at securing normal haemodynamics. No attempt was made to identify intracardiac lesions at this stage. Cineangiography two months to seven years later showed aorto-right ventricular fistulae in all patie...

Traumatic aortocaval fistula from gunshot wound, complicated by bullet embolization to the right ventricle

Aortocaval fistulas are most commonly the result of spontaneous rupture or erosion of an abdominal aortic aneurysm into the inferior vena cava (80-90%). The remaining 10-20% of aortocaval fistulas are usually the result of penetrating or iatrogenic trauma from gunshot or stab wounds. We report the case of a 19-year-old male, status post multiple gunshot wounds. To our knowledge, this is the fir...

Penetrating wounds of the heart and great vessels. Experience with 24 cases including 3 with intracardiac defects.

Twenty-four cases of penetrating cardiac injuries due to stab wounds, which were seen over a 3-year period, are reviewed. Ten patients were treated by aspiration alone because tamponade was mild, or because they were seen more than 12 hours after injury. One patient with multiple stab wounds in the ventricles and left atrium died during emergency thoracotomy in the admission room. The mortality...
